Application process for Reserve Force Compensation, Government Employees Compensation Act, or Extension of Class “C” Service (for service related injury or illness)

Reserve Force Compensation has three different processes depending on whether the member is in the Royal Canadian Army, Royal Canadian Navy or Royal Canadian Air Force.

  •          Army:  The application goes through the unit, the brigade, the division, and then to the Director Casualty Support Management.
     
  •          Navy: the application goes through the unit, Naval Reserve Headquarters, Royal Canadian Navy Corporate Services, and then to the Director Casualty Support Management.
     
  •          Air Force: the application goes through the wing, 1 Canadian Air Division, and then to the Director Casualty Support Management.
     

Government Employees Compensation Act

Application goes through the unit, to Employment and Social Development Canada, and then to the provincial Workers’ Compensation Board.

Class “C” Extension

The application goes through the unit directly to the Director Casualty Support Management.
 

Process Map for Extension of Class “C” Service for personnel whose injury or illness was sustained on Special Duty Service

For an extension of Class “C” service for personnel whose injury or illness was sustained on Special Duty Service, the injury or illness must occur during Special Duty Service.

  •          All forms must be completed before the end of Class “C” on Special Duty Service

The Responsible Unit Commanding Officer (in this case, the Employing Commanding Officer) must take the following steps:

  •          Inform the member’s home unit Commanding Officer
     
  •          Complete CF98 form  - Report of Injury, Disease or Illness

o   The CF 98 form is a reporting requirement of all injury or illness reports

o   Note that there are 14 days to report an injury to Director Casualty Support Management 2 for all service related injuries

  •          Complete DND 663 form – Hazardous Occurrence Report

o   Reporting requirement for all Hazardous occurrence

o   No Link to Extension of Class “C” Service

o   For workplace injury prevention purposes, the DND 663 form is sent to Director General Safety

  •          Refer the member to the Medical Officer, who will:

o   Assign Medical Employment Limitations/ Temporary Medical Category

o   Complete a Medical Report

o   Complete Annex “A” of the DND 2346 form, if applicable

  •          Complete the DND 2346 form - Application for extension of Class “C” Reserve Service for personnel whose injury or illness was sustained on Special Duty Service
     
  •          Provide a covering letter and recommendation

o   The Class “C” extension may be approved by email prior to the Director Casualty Support Management  receiving the DND 2346 form – provided the Medical Officer has completed Annex “A” of the DND 2346 form

Director Casualty Support Management:

  •          Will review the request for Class “C” extension
     
  •         Has the authority to approve Class “C” extension

o   The Class “C” extension can be approved up to a maximum of 24 months, however, an extension beyond 24 months may be approved on a case by case basis

Note: this process does not require several stages of approval within the chain of command.  The application goes from the employing commanding officer directly to the Director Casualty Support Management.

Reserve Force Compensation Process Map – Army

Member reports the service related injury or illness to the Unit Commanding Officer, who will:

  • Report the injury:
     
    • Complete CF 98 form – Report of Injury, Disease or Illness

- The CF 98 form is a reporting requirement of all injury or illness reports

- The CF 98 form is required for reserve force compensation claims (DND 2398 form)

- Note that there are 14 days to report an injury to Director Casualty Support Management 2 for all service related injuries

  •  
    • Complete DND 663 form – Hazardous Occurrence Report

- DND 663 form is used to report hazardous occurrences

- DND 663 form has no link to the reserve force compensation claim

- For workplace injury prevention purposes, the DND 663 form is sent to Director General Safety

  • The Joint Personnel Support Unit/Integrated Personnel Support Centre may provide assistance if required in completing these forms and reporting the injury

The Unit Commanding Officer refers the member to a Medical Officer, who will:

  •          Assign Medical Employment Limitations/ Temporary Medical Category
     
  •          Complete the Medical Report
     
  •          Complete Annex “A” of the DND 2398 form, if applicable
     

The Unit Commanding Officer will:

  •          Complete DND 2398 form – Reserve Force – Compensation during a Period of Injury, Disease or Illness
     
  •          DND 2398 form is only completed when there is an earning loss}
     
  •          The Medical Officer completes Annex “A” of this form
     

The Brigade will:

  •          Review the file
     
  •          Provide a covering letter and recommendation
     

The Division will:

  •          Review the file
     
  •          Provide a covering letter and recommendation
     
  •          May provide advance of funds up to 90 days
     

Director Casualty Support Management 2 will:

  •          Review and adjudicate the reserve force compensation claim
     

Director Casualty Support Management:

  •          Has delegated authority for the approval of reserve force compensation claims and advanced funds (90+ days)
     

Note: An application for extension of Class “C” service for personnel who sustained an injury or illness while on Special Duty Service goes from the employing commanding officer directly to the Director Casualty Support Management.

Process Map for Reserve Force Compensation– Navy

Member reports the service related injury or illness to the Unit Commanding Officer, who will

  •          Report the injury:

o   Complete CF 98 form – Report of Injury, Disease or Illness

- The CF 98 form is a reporting requirement of all injury or illness reports

- The CF 98 form is required for reserve force compensation claims (DND 2398 form)

- Note that there are 14 days to report an injury to Director Casualty Support Management 2 for all service related injuries

- DND 663 form has no link to the reserve force compensation claim

  •          Provide a covering letter and recommendation
  •          May provide advance of funds up to 90 days

Director Casualty Support Management 2 will:

  •          Review and adjudicate the reserve force compensation claim

Director Casualty Support Management:

  •          Has delegated authority for the approval of reserve force compensation claims and advanced funds over 90 days

Process Map for Reserve Force Compensation– Air Force

Member reports the service related injury or illness to the Wing Commanding Officer, who will

  •          Report the injury:

o   Complete CF 98 form – Report of Injury, Disease or Illness
 The CF 98 form is a reporting requirement of all injury or illness reports

- The CF 98 form is required for reserve force compensation claims (DND 2398 form)

- Note that there are 14 days to report an injury to Director Casualty Support Management 2 for all service related injuries

o   Complete DND 663 form – Hazardous Occurrence Report

- DND 663 form is used to report hazardous occurrences

- DND 663 form has no link to the reserve force compensation claim

- For workplace injury prevention purposes, the DND 663 form is sent to Director General Safety

  •          The Medical Officer completes Annex “A” of this form

1 Canadian Air Division Air Reserve will:

  •          Review the file
  •          Provide a covering letter and recommendation
  •          May provide advance of funds for up to 90 days

Director Casualty Support Management 2 will:

  •          Review and adjudicate the reserve force compensation claim

Director Casualty Support Management  

  •          Has delegated authority for the approval of reserve force compensation claims and advanced funds (90+ days)

Process Map for Government Employment Compensation Act

A member reports a service related injury or illness to the Unit Commanding Officer, who

  •          Reports the injury:

o   Completes CF 98 form – Report of Injury, Disease or Illness

- The CF 98 form is a reporting requirement of all injury/illness reports
 

- Note that there are 14 days to report an injury to Director Casualty Support Management 2 for all service related injuries
 

o   Completes DND 663 form – Hazardous Occurrence Report

- DND 663 form is used to report hazardous occurrences
 

- DND 663 form has no link to the reserve force compensation claim
 

- For workplace injury prevention purposes, the DND 663 form is sent to Director General Safety
 

o   Completes provincial employer reporting forms

- Must be completed within three days once the member informs the commanding officer that he or she will proceed with a claim under the Government Employees Compensation Act
 

- The provincial employer form includes a report of the injury and claim for compensation
 

- Once forms are complete they are sent to Employment and Social Development Canada.
 

The Unit Commanding Officer refers the member to a Civilian Medical Doctor/ Medical Officer:

  •          The member may choose to be seen initially by a civilian medical doctor or a medical officer. However, only a Canadian Forces medical officer can assign Medical Employment Limitations or a Temporary Medical Category.
  •          The Civilian Medical Doctor or Medical Officer then completes the Provincial medical form, which is sent to the Provincial Workers’ Compensation Board.

Employment and Social Development Canada will:

  •          Once it receives the provincial employer reporting forms, confirm the eligibility of the applicant
  •         If eligible for compensation under the Government Employment Compensation Act, Employment and Social Development Canada will send a request for compensation to the appropriate Provincial Workers’ Compensation Board.

Provincial Workers’ Compensation Board will:

  •          adjudicate and manage the Government Employment Compensation Act claim according to provincial legislation.
  •          This will be done once the provincial medical form (provided by Civilian Medical Doctor or Medical Officer) and the request for compensation (provided by the Employment and Social Development Canada) are received by the Provincial Workers’ Compensation Board.
