Before you provide us with your personal information, please take a few moments to review the following privacy statement. Links to the forms follow the text.

The Department of National Defence and the Canadian Forces (DND and the CAF) are committed to protecting the privacy rights of individuals and safeguarding the personal information under its control in accordance with the provisions of the Privacy Act.  “Personal information” is defined as any information, in any form, about an identifiable individual, including details of an individual’s personal experiences, opinions or views. Please refer to section 3 of the Privacy Act for further details regarding personal information.

The Personal information you are providing is collected for the purpose of the administration of suitable living accommodation as part of the Defence Administrative Orders and Directives DAOD 5024, DND Living Accommodation which is under the authority issued to CFHA by the Deputy Minister and the Chief of the Defence Staff.

The Department of National Defence is collecting your personal information which may include: name; contact information, date of birth, Canadian Forces or other identification number, financial information, employee personal information such as salary deductions, biographical information, employment equity information, financial institution information, and signature.

The Privacy Act states that you have the right to the protection of, access to, and correction of your personal information.  Details can be found in the following Personal Information Bank (PIB) Housing and Accommodations number DND PPU 885.

Refusal to submit the required information or consent, as requested for the purpose of the Department of National Defence Housing programs, will/may prevent the processing your request.

To access your personal information or notify us about incorrect information contact the Director Access to Information and Privacy (DAIP).

Formal requests for personal information are made under the Privacy Act and must:

  1. be made in writing and signed;
  2. identify the associated PIB number or provide sufficient information on the location of the personal information to render it reasonably retrievable; and
  3. be submitted to DAIP:
