What is the purpose of the form

The security screening certificate and briefing form (TBS/SCT 330-47) is used to security brief an employee after they have been granted a reliability status or security clearance. It is also used to terminate an employee's security screening when they leave your organization.

Who should complete the form

The company security officer (CSO) or the alternate company security officer (ACSO) should fill out the form.

Reference number

Leave blank. This is for use by the Contract Security Program only.

Departmental/organization number/file number

For initial briefings or changes in security requirements, these boxes are completed by the Contract Security Program.

For termination requests, you must enter your organization number in the “Departmental/Organization number” box. In the “File number” box, enter the individual's Personnel Identification Number. This is the 8-digit reference number that the Contract Security Program assigned to the individual. It usually starts with a number 9. If you do not know this number, you can leave that box blank.

Part A: Administrative information

For initial briefings or changes in security requirements, Part A is completed by the CSP.

For termination requests, the CSO must complete Part A.

Surname and full given names

Enter the individual's last name, and first and middle names, in the appropriate boxes. Do not use initials or nicknames. Underline the name they usually use.

Department/organization/agency

Enter your organization's legal name.

Unit/branch

Leave blank.

Initial briefing or reactivation and change in security requirement

When the CSP grants your employee a reliability status or security clearance, they complete and send you a security screening certificate and briefing form. When you receive this form, you must brief your employee on their new security roles and responsibilities. At the end of the briefing, your employee must sign the form. By signing, your employee acknowledges in writing that they agree to comply with the necessary security requirements.

After the briefing, you must also sign the form. You must keep all signed briefing forms in your organization's records and make them available to the CSP upon request.

Termination

When a person no longer works for your organization, you must submit a termination request to terminate their personnel security screening level. If the individual's security screening is still pending, you must also submit a termination request.

In both cases, you are responsible for completing the Security screening certificate and briefing form and sending it to the CSP.

Authorized level of individual

Select the personnel security screening level(s) held by the individual. If the personnel security screening you wish to terminate is pending, leave this section blank.

Signature and date

Leave blank.

Part B: Briefing summary

For initial briefings or changes in security requirements, review the briefing summary with your employee and answer any questions they may have. If you need help answering their questions, you can contact the Contract Security Program.

For termination requests, remind the individual of their security responsibilities and inform them that these remain in effect even after their reliability status or security clearance has been terminated.

Part C: Acknowledgement

For initial briefings or changes in security requirements, the employee must sign and date here. The date must be the date of their signature, not their date of birth.

For termination requests, the individual does not have to sign Part C.

Part D: Briefing official

For initial briefings, changes in security requirements and termination requests, you must complete Part D.

Print your name, initials and title. Lastly, sign and date the form.

Final steps

Place the original signed form in the individual's security file with the organization. For initial briefings or changes in security requirements, give a copy of the signed form to your employee.

Submitting the form

The CSO or ACSO must submit the Security screening certificate and briefing form to the CSP by fax, mail, courier or email. Access submitting request, forms and other documents for contract security to find out where to send your completed documentation.

The completed and signed form must be kept in the employee’s personnel file.
