Security assessments 6-month report: Controlled Goods Program
To comply with the Controlled Goods Program (CGP), registrants must complete a security assessments report every 6 months.
This report includes details regarding individuals who were security assessed by the designated official during the previous 6 months.

Submission requirements
Every 6 months, each registrant in the CGP, other than a sole proprietorFootnote 1, must submit a report of all individuals’ who were security assessed by the designated officials during the previous 6 months.
The registrant is responsible for submitting the report.
Sending in your first and following reports
The first report is due no later than 6 months from the registrant’s initial registration approval. Subsequent reports are due every 6 months thereafter.
Example
The company was approved on September 1, 2023.
- First report due: March 1, 2024
- Following report due: September 1, 2025
- Next report after that is due: March 1, 2025, and so on
Note
- you may email, mail or fax your report
- you do not need to sign a form to complete your submission
- you do not need to submit a report if you are a sole proprietor (that is, a one-person company with no other employees)
Preparing your security assessments 6-month report submission
Your submission must contain the following elements:
- the name of the authorized individual or designated official who submits the form, and their contact email address
- your business name and your Controlled Goods Program registration number
- include each individual in respect of whom the designated official conducted a security assessment during the previous 6 months
- if no security assessments were conducted during the last 6 months, include the statement "I acknowledge that no security assessments were conducted during the last 6 months" (the date we receive your email or fax will be considered the date of your declaration).
- for those with names to submit, please document the individuals in a table using the format prescribed below
We strongly encourage you to do provide large submissions in a spreadsheet.
Your report will have 6 columns. The headings are:
- surname
- given name
- date of birth
- date of security assessment
- risk level
- access granted

Description of report requirements
Please arrange your table in the order indicated in the sample.
- Surname (means last name or family name)
- Date of birth must be in numeric format YYYY-MM-DD (year-month-day)
- Date of security assessment must also be formatted YYYY-MM-DD
- The choices for Risk level are: low, moderate, high
- The choices for Access granted are either "yes" or "no"
Providing personal information
Public Services and Procurement Canada (PSPC) is committed to ensure the privacy and security of personal information. However, applicants remain responsible for the security and confidentiality of any personal information provided to PSPC, until it is received.
Applicants may choose to transmit personal or sensitive information, at their discretion, by:
- regular mail and courier
- fax
They should also consider using registered mail or priority courier as these are more secure delivery methods.
