Why has the form changed?
The new Background Check Consent Form provides additional information on candidates and their spouses that is critical for security, law enforcement and financial partners to conduct thorough verifications. This provides decision-makers with more information regarding a candidate’s suitability for appointment. Candidates for all Governor-in Council positions must complete the form.
Why does my spouse/partner need to sign the background check?
Information verified through the spousal check can provide further information on the candidate’s suitability.
What do I do if I do not have all the information?
Provide all the information you can, to the extent you are able. Include an explanation of why you were unable to complete the form (i.e. details unknown as not in contact with this individual). Use additional pages if necessary.
