Excel reporting form for responding to: Notice with respect to perfluorohexane sulfonic acid, its salts and its precursors (PFHxS)

Submitting your response to the notice is a two-step process that includes downloading and completing a PFHxS Excel Reporting Form and submitting the form via Environment and Climate Change Canada’s online reporting system. Any person who is required to respond to the notice must do so no later than November 20, 2020.

The Excel Reporting Form has been developed to assist you in meeting the reporting requirements of the notice. Please note that in the case of a discrepancy between the Excel Reporting Form, the Canada Gazette notice or the Canadian Environmental Protection Act, 1999 (CEPA), the official version of the notice and CEPA take precedence.
