PPC Framework Infographic
There are 3 major stakeholders in this model (which forms a triad of Responsibility):
- Member (Patient / Person)
- CFHS (Provider),
- and the Unit (Employer).
This model shows how each of these primary partners commits to the other 2 partners for this framework to be successful.

The specific responsibilities that each partner has to the others can be outlined while acknowledging that these relationships exist.
There is clear accountability within this framework while satisfying the health needs, operational readiness, and improved clinical outcomes for all stakeholders.
The foundation of this model comprises of the following guiding principles: Accountability, Respect and Dignity, Participation and Collaboration, Responsiveness, Belonging, Openness and Information Sharing:
- ACCOUNTABILITY – All persons, regardless of rank or role, hold other as “able” while they are accountable to each other to nurture working relationships.
- RESPONSIVENESS – Identifying, acknowledging, and responding to needs, trends and system changes.
- RESPECT AND DIGNITY – Every interaction is grounded in mutual respect and dignity that supports engagement.
- BELONGING – Every person works to create a culture of inclusion for all.
- PARTICIPATION AND COLLABORATION – Making spaces that support unified and collective goals for more successful outcomes.
- OPENNESS AND INFORMATION SHARING – Transparent sharing of information that is connected to decision-making.
