Forms – Endowment Incentives

The 2024-2025 Application Form has been separated into 4 sections :

• Part 1 - Foundation Form
• Part 2 - Arts Organization Form
• Part 3 - Joint Terms and Conditions
• Arts Organization - Resolutions of the Board of Directors

This change will allow foundations and arts organizations to complete their respective form without any limitations should they choose to complete it at the same time and will facilitate the collections of required signatures.

Only completed applications package comprised of Part 1 - Foundation Form, Part 2 - Arts Organization Form, and Part 3 - Joint Terms and Conditions sent with all requested supporting documents will be considered. Note that previous versions of the application form will not be accepted.
